    DR. PETER BAGLEY, Medical Director
    A pulmonologist and internal medicine physician, Dr. Bagley previously served as the Clinical Director of Fairlawn's Ventilator/Pulmonary Rehabilitation Program. He also serves as Director of the Intensive Care Unit at UMass Memorial Health Care, Memorial Campus, and as an Assistant Professor of Medicine at the University of Massachusetts Medical School, Worcester. A graduate of George Washington University School of Medicine, he is a Diplomate of the American Board of Internal Medicine for Pulmonary Diseases and Critical Care.

    DR. FAREN WILLIAMS, Chief of Physical Medicine and Rehabilitation
    Dr. Williams brings nearly 25 years of experience in physical medicine and rehabilitation (physiatry) to her role as Chief of Physical Medicine and Rehabilitation.
     
    Upon earning her medical degree at Wright State University School of Medicine, Dayton, OH, she completed an internship Internal Medicine at Christ Hospital, Cincinnati, and a residency in Physical Medicine and Rehabilitation at the University of Washington Medical Center, Seattle.
     
    Prior to joining Fairlawn's medical staff, she served as PM&R director at the Philadelphia Veterans Administration Medical Center and as an adjunct clinical associate professor at Jefferson Medical College in Philadelphia.
     
    Dr. Williams served on the medical staff of hospitals in Kentucky and Washington and has taught at numerous medical schools throughout the country. She has lectured and published extensively as well as co-authored publications on subjects related to physical medicine and rehabilitation.
     
    She is certified by the American Board of Physical Medicine and Rehabilitation and the American Board of Electrodiagnostic Medicine. She also has an MS in Nutrition and is a registered dietitian.

    DR. VALERIE VERIDIANO, Chief of Internal Medicine
    A Board certified internist and UMass-Memorial physician, Dr. Veridiano joined Fairlawn's medical staff in 1992. She earned her medical degree at the University of Santo Tomas in Manila, Philippines and completed her residency in Internal Medicine at St. Vincent Hospital in Worcester. She spent many years as a medical staff member at the Fallon Clinic. Dr. Veridiano is a member of the Massachusetts Medical Society and the Worcester County Medical Society.

    DR. DANIEL TANENBAUM, Physical Medicine and Rehabilitation Physician/ Outpatient
    A physical medicine and rehabilitation physician, Dr. Tanenbaum provides services to patients through our Outpatient Center. A graduate of Boston University School of Medicine, he is a Diplomate of the American Board(s) of Electrodiagnostic Medicine and Physical Medicine and Rehabilitation.

    DR. THU MALIKOWSKI, Physical Medicine and Rehabilitation Physician
    A board certified physical medicine and rehabilitation physician, Dr. Malikowski also serves on the medical staff of UMass Memorial Medical Center and as an assistant professor at UMass Medical School. A graduate of the University of Wisconsin Medical School, she completed her residency in PM & R through the University of Washington in Seattle. Prior to attending medical school, Dr. Malikowski earned her B.S. in Biology at Marquette University, where she was a member of the Phi Beta Kappa National Honor Society. A member of the American Academy of Physical Medicine and Rehabilitation, Dr. Malikowski is fluent in Vietnamese.

    DR. FRANCIS O'CONNELL, Director of Orthopedic Rehabilitation
    Dr. O'Connell brings more than 30 years of orthopedic surgical experience to the care he provides to Fairlawn patients. A graduate of Harvard University, he earned his medical degree at the Columbia University College of Physicians and Surgeons. Dr. O'Connell is a member of the Orthopedic Department at UMass Memorial Health Care and currently serves as an Assistant Professor of Orthopedic Surgery at the University of Massachusetts Medical School.

    DR. ELIZABETH ROAF, Physiatrist
    Dr. Roaf, who previously served as Fairlawn's Medical Director, is board certified in Internal Medicine, Physical Medicine and Rehabilitation, and Spinal Cord Injury / Disease Medicine. A graduate of the University of Vermont's College of Medicine, she is an assistant professor of Rehabilitation Medicine at the University of Massachusetts Medical School. Dr. Roaf is a member of the Massachusetts Medical Society.

    DR. KEVIN SULLIVAN, Physiatrist
    A board certified physical medicine and rehabilitation physician (physiatrist), Dr. Sullivan specializes in treating individuals with spine problems, including acute and chronic neck and back pain, degenerative conditions, spinal stenosis, herniated discs, and spinal trauma. Dr. Sullivan practices Physiatry with the Boston Spine Group whose offices are located in the New England Baptist Hospital. He also serves on the medical staff at Metro-West Medical Center, Framingham, MA, and maintains an office in Southboro, MA. A graduate of New York Medical College, Valhalla, NY, he completed a residency in Physical Medicine and Rehabilitation at Harvard Medical School / Spaulding Rehabilitation Hospital and a fellowship in Musculoskeletal and Pain Medicine at the Georgia Pain Physicians in Marietta, GA. He also completed an Internal Medicine internship at Harvard Medical School / Beth Israel Deaconess Medical Center, Boston. He is a member of the American Academy of Physical Medicine and Rehabilitation, the Physiatric Association of Spine, Sports, and Occupational Rehabilitation, International Spinal Injection Society, and the North American Spine Society. Prior to entering medical school, Dr. Sullivan played professional hockey with the San Jose Sharks.

    DR. NELLIE DORMAN, Internal Medicine Physician
    Dr. Dorman's patients benefit from her dual expertise in Physiatry and Internal Medicine. Having served on Fairlawn's medical staff since 1989, Dr. Dorman earned her medical degree at the Moscow Medical Institute and completed her residency in Physical and Rehabilitation Medicine at the Moscow Scientific Research Institute of Health Resorts and Physical Therapy. In addition to receiving medical training at Spaulding Rehabilitation Hospital and the Veteran Administration Medical Center in Boston, she completed a residency in Internal Medicine at the former Worcester City Hospital.

    DR. AFROZ SAQUIB, Internal Medicine Physician
    Dr. Afroz Saquib, a board certified Internist, became a member of Fairlawn's medical staff after completing her residency in Internal Medicine at UMass Memorial Medical Center, Worcester. Upon earning her medical degree from Nagpur University, Nagpur, India, she completed internships at several hospitals in India. Prior to her residency training, Dr. Saquib spent two years at Massachusetts General Hospital, Boston, doing research on Hepatitis C Virus.

    DR. DURGA BATHINI, Internal Medicine Physician
    A Board certified internist, Dr. Bathini earned her medical degree at Kakatiya Medical College in Warnagal, India. She then served as a Locum Intern for the Ministry of Health at the University of West Indies Hospitals in Kinston, Jamaica. Dr. Bathini completed her residency in Internal Medicine at UMass Memorial Healthcare and previously served on the medical staff of Primary Care at the Community Health Center at Tyndall Air Force Base in Panama City, Florida. She is a member of the Massachusetts Medical Society and the American Medical Association.

    DR. MARK FINNO, Physiatrist
    A Physical Medicine and Rehabilitation physician at Fairlawn's Outpatient Center, Dr. Finno specializes in treating patients with back and neck injuries. Upon earning his medical degree at St. George's University School of Medicine, St. George's, Grenada, he completed an internship in Internal Medicine at Staten Island University Hospital, Staten Island, NY. He then completed a residency in Physical Medicine and Rehabilitation at New York Presbyterian Hospital / Cornell Campus and served as Chief Resident in his final year. Dr. Finno completed a fellowship in Physical Medicine and Rehabilitation at New England Baptist Hospital's Spine Center, Boston, and is now a member of the Boston Spine Group. Actively involved in community programs, he has served as a volunteer for the National Veterans Wheelchair Games and the New York City Marathon.

    DR. DAVID KENT, Neuropsychologist
    Dr. Kent has provided psychological services to Fairlawn patients and families since 1988. Currently, he oversees the clinical and administrative operations of the hospital's Neuropsychology Services. In addition to maintaining a private practice, he serves as a consulting psychologist at St. Vincent Hospital's Center for Psychiatry and at UnumProvident Corporation. Dr. Kent earned his doctoral degree in clinical psychology at Indiana State University. Following completion of his academic coursework, he completed an internship in Clinical Psychology at the VA Medical Center in Northport, NY. Dr. Kent also completed two post-doctoral fellowships in Rehabilitation and Neuropsychology. His initial fellowship was at St. Charles Hospital and Rehabilitation Center in Port Jefferson, NY. His second fellowship was at Fairlawn Rehabilitation Hospital and Worcester State Hospital. Dr. Kent is a member of the American Psychological Association, the National Academy of Neuropsychology, the Massachusetts Neuropsychological Society, and the National Register.

    DR. SHEREE L. ESTES, Neuropsychologist
    Dr. Estes provides neuropsychological and psychological services as well as psychotherapy for Fairlawn's patients and their families. She earned her doctorate in Clinical Psychology at Forest Institute of Professional Psychology, Springfield, MO, and her master's degree in Guidance and Counseling at Southwest Missouri State University, Springfield, MO. Prior to joining Fairlawn in 1999 as a neuropsychology resident, Dr. Estes served as a neuropsychology fellow at St. Charles Hospital and Rehabilitation Center in Port Jefferson, NY. A member of the American Psychological Association, she has provided therapeutic services in a variety of settings serving individuals with diverse diagnoses.

    DR. DAVID NOWELL, Neuropsychologist
    A clinical psychologist with extensive experience in treating individuals with brain injury, Dr. Nowell provides neuropsychological and psychotherapy services for Fairlawn patients and their families. Along with maintaining a private practice in Worcester, he also serves as a consultant for HealthSouth Rehabilitation Hospital of Western Massachusetts, the Statewide Head Injury Program in Boston, the Carson Center for Traumatic Brain Injury Services of Central Massachusetts, and St. Vincent Hospital, Worcester. Dr. Nowell earned his doctorate degree in Clinical Psychology at the University of Alabama. He completed a predoctoral internship in Neuropsychology at Tulane University Medical Center's School of Medicine, New Orleans, LA, and a postdoctoral clinical neuropsychology fellowship at Fairlawn. He is a member of the American Psychological Association.

    PAULA ESPER, Nurse Practitioner
    Paula has served as a nurse practitioner in Fairlawn's Orthopedic Rehabilitation Program since 1996. She is a graduate of the University of Massachusetts Medical School / Graduate School of Nursing, where she earned her MSN / Acute Care Nurse Practitioner degree. She also serves as coronary care nurse at UMass Memorial Medical Center in Worcester.

    LETTY WHEELOCK, Nurse Practitioner
    An Adult Nurse Practitioner who specializes in treating geriatric patients, Letty works very closely with Fairlawn's Internal Medicine physicians. Prior to joining Fairlawn's medical staff, she served as a Geriatric Nurse Practitioner for the Fallon Clinic and as the Executive Director and Nurse Manager of the Adult Day Health Center at the Beaumont Skilled Nursing Facility, Northbridge. She also previously worked as a staff nurse at St. Vincent Hospital, Worcester, and the VNA of South Central Massachusetts. Letty earned her MS in Nursing/Adult Practitioner degree at the University of Massachusetts Graduate School of Nursing, Worcester, and her BS in Nursing at Assumption College.
