Direct Access Admission Process
Fairlawn Rehabilitation Hospital's Direct Access Admission Process provides patients with rapid access to a rehabilitation program by allowing them to be referred directly to Fairlawn by their physician, from home, hospital emergency rooms, or community agencies .
Goals
Designed for individuals who are unable to remain at home due to a decline in their medical and/or functional status, the program focuses on:
- Managing Medical Needs
- Maximizing Functional Abilities
- Assessing Community Support
- Facilitating Safe Return to the Community
Program Components
Each patient engages in a specialized rehabilitation program based on his or her medical diagnosis and rehabilitation needs.
Program components include:
- Comprehensive Medical Management
- Therapeutic Exercise Program
- Functional Approach to Activities of Daily Living/Community Re-entry
- Patient/Family Education and Support
- Consulting Services (i.e., speech therapy, neuropsychology, vocational counseling)
- Weekly Team Meeting
- Family Conference (as needed)
- Ongoing Collaboration With Referring Physician
- Case Management
- Comprehensive Outpatient Services
